Promo Mechanics
1. Promo period is from February 1 to 29, 2024.
2. Doctor Anywhere (DA) Maxicare Members who have an active DA account and are prescribed by the DA doctors to undergo laboratory and/or imaging test/s during the campaign period are entitled to join the raffle.
3. DA Maxicare Members will receive their e-referral document via the Doctor Anywhere app and email. They shall avail of the prescribed laboratory and/or imaging test/s in a Maxicare Primary Care Center or Maxicare Home Care Service within the campaign period.
3. For the raffle, Doctor Anywhere will be checking its February 2024 utilization report and the February 2024 Maxicare Primary Care Center and Home Care Service availment data to determine who shall be part of the raffle.
4. Deadline for availment is on February 29, 2024, at 11:59 PM.
5. Raffle draw or determination of winners will be held on March 7, 2024, at 2:00 PM, in the presence of a DTI representative.
6. Prizes are ₱1,000 GCash credits each for the 15 consolation prize winners and ₱100,000 GCash credits for the 1 major prize winner.
7. Tax (including 20% withholding tax for prizes exceeding P10,000.00) and other expenses that will be incurred in relation to the prize shall be shouldered by Doctor Anywhere Philippines Inc.
8. The prizes are not transferable and cannot be exchanged for nor redeemed as cash or other items.
9. Winners shall be notified via registered mail and email.
10. A DA member cannot win multiple times.
11. The winner may claim his/her prize via GCash.
12. To claim the prize, winners must answer the Google Form sheet that we’ll send for proper verification.
13. If claiming through an authorized representative, present two valid government-issued IDs of the winner and an authorization letter. This letter should be prepared and signed by the winner, granting permission to the authorized representative to claim the prize.
14. Prizes should be claimed within sixty (60) days from the receipt of registered notice or they will be forfeited in favor of Doctor Anywhere Philippines, Inc. with prior approval of DTI.
15. All employees of Doctor Anywhere Philippines, Inc., its affiliates, and their relatives up to the second degree of consanguinity or affinity are disqualified from joining this promo.
