Promo Mechanics

 

1. Promo period is from March 1-31, 2024.

 

2. Any individual who completes Doctor Anywhere’s women’s health quiz is entitled to join the raffle.  During the promo period, one completed quiz response is equivalent to one raffle entry. An individual is eligible for only one raffle entry, regardless of how many times they answer the quiz.

 

3. For the electronic raffle - quiz respondents must completely fill out their personal information at the end of the quiz with the following details: complete name, contact number, postal/home address, and email address.

4. The deadline for submission of entries is on March 31, 2024, at 11:59 PM. 

 

5. Raffle draw or determination of winners will be held on April 2, 2024, at 10:00 AM in the presence of a DTI representative. 

 

6. Raffle prizes are ₱200 Grab Voucher each for 500 winners. In addition to earning a raffle entry, all quiz participants will also receive a ₱30 discount promo code (WOMENSHEALTHGP) for their video consultation with a general practitioner. This discount code is valid for individuals with a Doctor Anywhere account and no health plan subscription or for those who have not linked their HMO or insurance accounts to Doctor Anywhere. The discount code is not convertible to cash and is applicable for a one-time use per account, valid only for a video consultation with a general practitioner until March 31, 2024.

 

7. Tax (including 20% withholding tax for prizes exceeding P10,000.00) and other expenses that will be incurred in relation to the prize shall be shouldered by Doctor Anywhere Philippines Inc.

 

8. The prizes are not transferable and cannot be exchanged for nor redeemed as cash or other items.

 

9. Winners shall be notified via registered mail and email that includes their unique Grab voucher code to redeem their prize via the Grab app, and their names will be posted on Doctor Anywhere Philippines' official Facebook and Instagram accounts.

 

10. An individual can only win once.

 

11. The winner may claim his/her prize via the Grab app by using the unique voucher code sent to their registered mail and email. 

 

12. If claiming through an authorized representative, present two valid government-issued IDs of the winner and an authorization letter. This letter should be prepared and signed by the winner, granting permission to the authorized representative to claim the prize on their behalf.

 

14. Prizes should be claimed within sixty (60) days from the receipt of registered notice or they will be forfeited in favor of Doctor Anywhere Philippines, Inc. with prior approval of DTI. 

 

15. All employees of Doctor Anywhere Philippines, Inc., its affiliates, and their relatives up to the second degree of consanguinity or affinity are disqualified from joining this promo.
